This article explains what the international Whole-Body EMS guideline means for commercial providers.
The guideline reflects the current international scientific and professional consensus on safe commercial application. It is not a worldwide law and does not replace local regulations or the instructions for the specific device.
Its principles are already reflected in professional requirements and regulations in some markets. They provide an important framework for providers, manufacturers, education providers and companies developing Whole-Body EMS business concepts.

Why Is There an International Guideline?

Whole-Body EMS differs from conventional exercise because electrical stimulation activates several major muscle groups simultaneously.
This is the main advantage of the method. A relatively short session can create an effective whole-body training stimulus.
It is also the main reason why the application needs clear professional standards.
If the intensity is too high, the session is too long or recovery is too short, the participant may be overloaded. The effects may not be fully apparent during the session itself. Muscle soreness and other signs of excessive strain can become more noticeable hours or days later.
This delayed response makes self-assessment difficult. A participant cannot rely only on how the session feels at that moment.
Reports of adverse effects following inappropriate application led researchers to publish the first safety recommendations in 2016. As the international market grew and new concepts appeared, those recommendations were updated.
The result was the 2023 publication:
Position Statement and Updated International Guideline for Safe and Effective Whole-Body Electromyostimulation Training – The Need for Common Sense in WB-EMS Application.
The guideline was developed through an international consensus process involving research groups from Brazil, Germany, Italy, Spain and the United States.
The authors include experts in exercise science, physiology, rehabilitation and cardiovascular medicine, including specialists from the Department of Cardiovascular Medicine at Mayo College of Medicine.
The guideline focuses on commercial, non-medical Whole-Body EMS Training.

What Does the Guideline Require?

The most important requirement is close personal supervision.
One-to-one supervision is preferred. In non-medical training, one qualified trainer may supervise two suitable participants when full individual control remains possible.
The trainer must be physically present and close enough to:
•    observe each participant
•    communicate without difficulty
•    adjust the stimulation individually
•    correct movement
•    reach the controls immediately
•    stop the application without delay
The trainer must focus on the participants throughout the active session. Starting a device and then moving on to another task does not meet this standard.
The guideline also requires providers to consider:
•    trainer qualification
•    health and contraindication screening
•    the participant’s condition before each session
•    reduced intensity and duration for beginners
•    gradual progression
•    individual intensity control
•    sufficient recovery
•    immediate access to the device controls
For intensive strength-oriented Whole-Body EMS Training, the active application should not exceed 20 minutes.
During the first 8–10 weeks, intensive sessions should take place no more than once per week. After this introductory period, at least four days should separate intensive sessions.
Training to complete exhaustion should be avoided.
Good Whole-Body EMS Training is not about maximum intensity.
It is about applying the right stimulus, stopping at the right time and allowing sufficient recovery.

What This Means for the Business Model

Professional Whole-Body EMS Training is a supervised service, not simply access to a device.
The customer receives more than technology. The service includes health screening, equipment setup, individual intensity control, continuous observation and adaptation of the session.
This has direct consequences for the business model.

Staffing
Every active session requires a qualified trainer who is physically present.
One trainer should supervise no more than two suitable participants. Providers must therefore plan staffing around the number of participants training at the same time.

Capacity
A provider cannot increase capacity simply by assigning more participants to one trainer.
Professional growth should come from:
•    additional qualified trainers
•    additional training stations
•    efficient appointment processes
•    longer opening hours
•    additional locations
The operating principle is:
Scale through qualified capacity, not through less personal supervision.

Pricing
Pricing must cover the complete supervised service.
This includes trainer time, screening, setup, individual control, documentation, hygiene, maintenance and professional operating procedures.
Whole-Body EMS should therefore be positioned as technology-supported personal training rather than low-cost equipment access.

Technology and Innovation
Digital tools can support booking, documentation, customer communication and business administration.
They do not remove the need for personal supervision during the application.
Not every concept that looks modern or highly scalable provides the level of individual control required for safe professional use.
Every concept should be assessed against one question:
Does it maintain qualified personal supervision and full individual control for no more than two participants per trainer?

What Does the Guideline Mean Legally?

The international guideline is not a law.
However, it represents the main international scientific and professional framework for safe commercial Whole-Body EMS application. Its principles can also support the development of national regulations and professional standards.
Serious providers, manufacturers, education providers and business concept developers should therefore orient their safety concepts around it.
A company may choose a different approach. It should then be able to show how it achieves an equally strong level of qualification, screening, individual control and emergency response.
If an incident occurs, scientific recommendations, device instructions, local standards and documented operating procedures may become relevant when assessing whether reasonable care was taken.
An unexplained departure from the guideline may therefore increase legal, insurance and reputational risk.
